Participants under the age of 19 must have a parent/guardian complete this form.   Participation in PianoFest is contingent upon receipt of the full registration fee per participant. Payment must be paid by check (made to "UNO School of Music/PianoFest") and sent to Dr. Kristín Jónína Taylor at the following address: Maverick PianoFest, c/o Dr. Kristín Jónína Taylor, School of Music, University of Nebraska at Omaha, 6001 Dodge St., Omaha, NE 68182.   Please indicate the name of the student on the memo line of the check.
